Memory Support Pathway- Porthmadog Dementia Centre 

Here’s sharing information about a new project called the Dementia Centre (which falls under the Memory Support Pathway) at Byw’n Iach Glaslyn, Porthmadog on the second Friday of every month. Dementia Actif have been collaborating with Siân Williams, the Dementia Centre Coordinator, to develop this exciting program, and we look forward to offering additional support to people affected by dementia in the community.
